International Students Travel Form - Host Family Signature

I affirm that both I and the legal parent(s) of above student have given permission to said student to attend the trip described in this form. I have verified and confirmed all necessary transportation, accommodations, and oversight for the entire duration of the trip for said student. the legal parent(s)/guardian(s) and I have also verified that said student will be accompanied by the responsible adult over the age of 21 while on the trip. The legal parent(s) agree to take full responsibility for the safety and well being of said student on this trip while away from the host family.
